Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver patient-centered care in home settings under the supervision of a Physical Therapist. You will implement treatment plans, document progress, and participate in team in-services to optimize patient outcomes.

The role requires a valid PT license, CPR certification, and strong geriatric care knowledge, with preferred experience as a PTA.

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
Read, write, and speak fluent English.
Preferred: One year of experience as a Physical Therapy Assistant.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes.
Administer non-complex manual therapies and modalities as directed.
Document client records and submit clinical documentation weekly.
Attend educational meetings and coordinate department in-services.
Notify Physical Therapist or Branch Director of equipment issues or changes.
Apply infection control and universal precautions in patient care.
