Job summary

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Excellent communication and writing skills.

Responsibilities

• Provides high-quality clinical services within the scope of practice, per plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and reports changes in patient condition timely.
• Must be able to function in diverse home environments and populations.
