Job summary

UnitedHealth Group through Elk Valley, part of LHC Group, is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes as prescribed by physicians and to document care and coordinate other patient activities. You will work under the supervision of a Registered Nurse or supervisor to ensure the highest quality of care.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Current driver's license and access to a dependable vehicle or public transportation.
• Flexibility to work varied hours to meet client needs.
• Ability to work independently.

Responsibilities

• Provides high quality clinical nursing services per plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates changes in patient condition.
• Function effectively with diverse patients and settings.
