Job summary

Elk Valley, part of LHC Group and a member of the Optum family, is seeking a Licensed Practical Nurse to provide direct nursing care for acutely and chronically ill patients in their homes under the supervision of a Registered Nurse. The role includes documenting care and coordinating services within the geographic service area.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Valid driver's license and access to reliable transportation.
• CPR certification preferred or within 90 days of hire.

Responsibilities

• Provide high quality clinical nursing care per physician plan and state practice laws.
• Document visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Coordinate patient care activities and communicate with patient, family, physician and care team.
• Function effectively in diverse home care settings and with diverse patient populations.
