Job summary

Elk Valley, a part of LHC Group, is hiring a Licensed Practical Nurse to provide direct home health nursing care as prescribed by physicians, in the patients' homes within the assigned geographic area. You will document visits, coordinate care, and work under supervision of a Registered Nurse or supervisor.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Preferred CPR certification or ability to complete within 90 days of hire

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patient, family, physician, and care team.
• Able to function in any home situation regardless of client background.
