Job summary

Elite Home Health, part of the LHC Group and Optum family, seeks a Licensed Practical Nurse to provide quality patient care in patients' homes. You will perform skilled nursing tasks, document visits, and collaborate with the care team to ensure the plan of care is followed.

Responsibilities include care coordination, accurate charting, and maintaining infection control standards, with flexible hours to meet patient needs in a home health setting.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and the plan of care.
• Documents patient visits in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reports patient needs and changes in condition, and communicates with patient, family, physician, and care team.
• Function in diverse home situations and serve patients regardless of age, race, creed, color, sex, disability, or financial condition.
