Job summary

UnitedHealth Group, including Elk Valley home health, is seeking a Licensed Practical Nurse to deliver direct nursing care in patients'' homes under physician orders. You will document visits and coordinate care with the health team to ensure high-quality service and patient comfort.

The role requires an active LPN license, valid driver's license and access to a vehicle. Flexible hours, supportive supervision, and opportunities for growth are available.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver''s license and access to a vehicle or reliable transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Provides high quality clinical nursing services as prescribed by the physician, in patients'' homes.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Coordinates patient care activities with the health care team and case managers.
• Functions effectively in diverse home settings regardless of patient age, race, or background.
