Optum's Elk Valley division is hiring Licensed Practical Nurses to provide direct nursing care in patients' homes within the Tennessee service area. You will work under supervision and coordinate with the health care team to deliver high-quality care.

You will document visits in the electronic medical record, participate in care coordination, and adapt to diverse home environments. Required qualifications include LPN licensure and valid driver's license; CPR preferred and flexibility in hours.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance; access to a dependable vehicle.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to work flexible hours and manage multiple tasks.

Responsabilités

• Provide direct nursing care per physician's orders in patients' homes.
• Document visits and progress notes in the electronic medical record.
• Coordinate patient care activities and communicate with families and clinicians.
• Maintain safety and infection control procedures in home settings.
