Job summary

UnitedHealth Group’s Elk Valley unit is hiring a Home and Community Based Services Licensed Practical Nurse to deliver direct nursing care in patients’ homes under physician guidance. You will document visits, coordinate services, and communicate with patients, families, and the health team to ensure high-quality care in your service area.

Requirements include an active LPN license, a valid driver’s license with reliable transportation, and the ability to work flexible hours.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical nursing services in the home per physician plan.
• Documents patient visits in the electronic medical record and supports care coordination.
• Communicates patient needs and changes in condition with the care team and case managers.
• Functions effectively in diverse home environments and with diverse patients.
