Job summary

Elk Valley, part of LHC Group and within the Optum family, seeks an Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes under physician orders, and record clinical notes in the EMR.

You will work under supervision to coordinate care activities, ensuring high-quality patient care within local service area and adapting to varied home environments.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and the plan of care, coordinating with health care team.
• Documents patient visits thoroughly and per regulatory and payer requirements in the electronic medical record.
• Participates in the agency’s care coordination process, communicating with patient, family, physician, clinicians, and case coordinators.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
