Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care services in patients' homes in accordance with an established care plan.

Responsibilities include assisting with baths, hygiene, nutrition, transfers, ROM exercises, safety, and documenting observations for the supervising nurse. Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements

Responsibilities

• Assist patients with daily living according to plan of care (baths, Toileting, dressing, feeding, etc.)
• Take and record vital signs as ordered (oral, axillary, etc.)
• Assist with safe transfers and ambulation
• Provide range of motion and exercise as ordered
• Perform basic tasks per plan (laundry, safety measures)
• Observe and document patient condition and report changes to nurse supervisor
• Communicate clearly with client, family/caregiver and office
