Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established care plan.

The role includes baths, mobility assistance, and routine monitoring of health status as directed by the plan of care. Responsibilities include documenting patient conditions, communicating with family and supervisors, and adhering to safety practices.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements

Responsibilities

• Assists with baths, oral hygiene, dressing, toileting, feeding as per plan of care
• Takes and records vital signs as ordered
• Assists with safe transfers and ambulation
• Exercises per plan of care and assists with range of motion
• Performs simple procedures as ordered
• Performs patient laundry and maintains safety practices
• Observes and documents patient condition and notifies nurse
