Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes under an established care plan. Duties include daily living tasks, personal care, and basic health monitoring as directed by the plan of care.

You will document care, communicate with families and the supervising nurse, and help ensure patient safety during daily activities. A part-time schedule with flexible hours and competitive pay is offered.

Qualifications

• Meets training requirements per state/federal laws as applicable.
• Able to read, follow written instructions and document care.
• CPR certification maintained per state regulations.
• Complete Maxim screening examinations and competency program as required.

Responsibilities

• Assist patients with activities of daily living per care plan.
• Perform baths, grooming, dressing, toileting and feeding assistance as directed.
• Take and record vitals when ordered.
• Assist with safe transfers and ambulation.
• Support range of motion and simple therapy activities as ordered.
• Handle patient laundry and maintain safety using equipment.
• Observe and report patient condition changes to supervising nurse.
