Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes with daily living activities and personal care according to an established care plan. The role emphasizes safety, dignity, and clear communication with the care team.

The position is part-time with flexible hours and competitive pay. You will work under supervision from a licensed nurse and contribute to maintaining patients’ independence at home.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements

Responsibilities

• Assist patients with daily living activities according to the plan of care.
• Monitor and record vital signs as ordered and report changes to the supervising nurse.
• Support safe transfer and ambulation per care plan.
• Provide personal care tasks and hygiene assistance as directed.
• Perform range of motion or simple procedures as qualified.
• Document care provided and communicate with client, family, and office.
• Maintain patient safety and home safety practices and use equipment properly.
