The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients at home according to established care plans.

Responsibilities

• Assist patients with daily living tasks per plan of care (bathing, oral hygiene, dressing, toileting, feeding).
• Take and record vital signs when ordered (temperature, pulse, respiration, blood pressure).
• Assist with safe transfers and ambulation as directed.
• Follow plan of care and complete all assigned tasks.
• Observe, document patient condition and report changes to the supervising nurse.
• Provide patient and family education on home safety practices.
