Job summary

The Lakes Home Care Services is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes following an established care plan. You will support baths, grooming, feeding, and safety while documenting care and communicating with the nursing team.

Responsibilities include vital sign checks, safe transfers, observation, and adherence to the care plan. Flexible hours and a competitive salary are offered.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with activities of daily living per plan of care (baths, dressing, feeding, etc.).
• Take and record vital signs as ordered.
• Assist with safe transfers and ambulation.
• Assist with range of motion and simple procedures as ordered.
• Handle patient laundry and daily needs.
• Ensure safety and proper use of equipment.
• Observe and document patient condition and report changes to nurse.
• Communicate clearly with client, family/caregiver and office.
