The Lakes Home Care Services is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes in accordance with an established care plan.

The role involves assisting with baths, dressing, mobility, safety, and documentation, while adhering to plan of care and reporting any changes to the supervising nurse. Join a dedicated team focused on quality patient care.

Qualifications

• Must follow the plan of care and document care given.
• Ability to read and follow written instructions.
• Maintains CPR certification as required by state law.
• Completes Maxim screening examinations and competency program.

Responsabilités

• Assist patients with daily living activities as per plan of care (bathing, oral hygiene, dressing, toileting, feeding).
• Take and record vital signs as ordered and document results.
• Assist with safe transfers and ambulation.
• Perform ROM and simple procedures as ordered.
• Care for patient laundry and maintain safety practices in the home.
• Observe and report patient condition to supervising nurse.
