Job summary

Heart N Home Hospice, part of the Optum family, invites you to join as a Home and Community Based Services Aide, helping clients stay safely in their own homes with personal care and supportive daily tasks. You will bathe, groom, assist with mobility, prepare meals, document observations in EVV, and accompany clients to appointments.

CPR certification is preferred, 6+ months home care experience, and you must have reliable transportation.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation
• 6+ months of home care experience
• Current CPR certification or ability to complete within 90 days of hire
• Ability to work flexible hours and independently

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
