Elite Hospice, a part of LHC Group, is seeking a Home and Community Based Services Aide to provide in-home care and support for clients to remain in their own homes. Responsibilities include personal care, daily living activities, and accompanying clients to appointments, with emphasis on safety and compassionate care.

The role requires a valid driver's license and access to reliable transportation; state-specific MO requirements apply.

Qualifications

• Provide personal care and assist with daily living activities.
• Monitor and document client condition and vital signs.
• Ensure safe home environment and operate basic medical equipment.

Responsabilités

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
