Elite Hospice, a part of LHC Group, is hiring a Home and Community Based Services Aide to assist clients at home with personal care and daily living tasks. You will support routines, monitor well-being, and help maintain a safe home environment.

Responsibilities include documentation, using EVV, and coordinating transportation for appointments while upholding dignity and confidentiality. Ability to work independently and flexibly is essential.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation
• 6+ months of home care experience
• Ability to work flexible hours and independently

Responsabilités

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
