Job summary

Assured Hospice is seeking a Home and Community Based Services Aide to provide personal care and assist with daily living activities in a client's home, helping them live comfortably at home. You’ll support household tasks, meals, appointments, and monitor health, documenting with EVV while ensuring confidentiality and safety.

The role requires a valid driver’s license, CPR (or ability to obtain within 90 days), and 6+ months of home care experience, with flexible hours and the ability to work

Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
