Job summary

Elite Hospice, part of LHC Group and within the Optum family, is seeking a Hospice Home and Community Based Services Aide to help clients remain in their own homes. You will provide personal care, assist with daily activities, monitor health, and support families in a compassionate, professional setting.

TX-specific requirements include home health experience, CPR certification (or ability to obtain within 90 days), a valid driver's license, and reliable transportation.

Qualifications

• Must have a current driver's license, vehicle insurance, and reliable transportation or access to public transportation.
• State-specific requirements include TX home health experience or nursing student competency as applicable.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal preparation, and accompany clients to appointments.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment and operate medical equipment; respond promptly to client needs.
