Job summary

Gentiva Hospice is hiring an RN Case Manager to deliver compassionate home-based hospice and palliative care. You will assess patients, develop and adjust care plans, coordinate with the IDG, and educate families on end-of-life care.

Applicants should hold an active RN license, have hospice experience, and be CPR-certified. This role involves travel to patients' homes within the service area and requires strong communication and teamwork.

Qualifications

• Must hold a valid RN license in the state of employment.
• Hospice or palliative care experience preferred.
• CPR certification required and maintained.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with interdisciplinary teams and physicians.
• Educate patients, families, and caregivers on disease, self-care, and end-of-life options.
• Assist with on-call duties and respond to patient needs promptly.
• Document all clinical information and ensure compliance with policies.
