Job summary

InnovAge is seeking an In-Home Services Registered Nurse to provide skilled nursing care in participants’ homes or InnovAge facilities. The role includes triaging calls, scheduling, administering medications, documenting care, and coordinating with the Home Care team to ensure appropriate levels of care.

The RN will instruct participants and families, perform assessments, and participate in quality management activities. A valid RN license, BLS, and 1+ year geriatric experience are required.

Qualifications

• Associate’s degree in nursing.
• Current State issued Registered Nurse license.
• Current First Aid and BLS certifications are required prior to hire.
• Require personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law.
• 1+ year working with the frail or elderly.

Responsibilities

• Triages participant and staff calls to determine priority of care and routes accordingly.
• Schedules outside appointments for participants when necessary.
• Assists in examination, testing and treatment of participants.
• Implements orders written by primary care physician(s).
• Instructs participants and family regarding medications and treatment instructions.
• Schedules specialist and other appointments for participants and assists in hospital admissions.
• Performs nursing care including administering medications and treatments.
• Documents participant information and maintains medical records and reporting requirements.
• Administers medications and trains participants in self-administration.
• Provides health counseling to participants and families.
• Assesses participants to determine changes in condition and level of care.
• Coordinates plan of care with Home Care resources.
