Zusammenfassung

InnovAge is seeking an In-Home Services RN to provide skilled nursing care in accordance with InnovAge policies, in participants' homes or at InnovAge facilities. The role involves triaging calls, scheduling appointments, administering medications, and coordinating care with the Home Care team.

Requirements include an Associate’s degree in nursing, current RN license, First Aid and BLS certifications, and 1+ year of elderly care experience.

Qualifikationen

• Associate’s degree in nursing.
• Current State issued RN license.
• First Aid and BLS certifications required prior to hire.
• Personal transportation and valid driver’s license with good driving record.

Aufgaben

• Triage participant and staff calls to determine priority of care.
• Schedule outside appointments for participants as needed.
• Administer prescribed medications and treatments, and provide nursing care in participants' homes or at facilities.
• Document medical records and complete charting per agency policy.
• Coordinate plan of care with Home Care resources and supervise LPNs/HHA/CNA per regulatory requirements.
• Provide health counseling to participants and families.

Kenntnisse
