Job summary

InnovAge is seeking a Universal RN to provide skilled nursing care in clinic and in-home settings, collaborating with the IDT to develop and execute Plans of Care for frail and elderly participants.

This role requires in-person presence at InnovAge centers or participant homes, adherence to HIPAA and safety standards, and active engagement in care coordination and documentation to ensure high-quality, compliant patient care. Travel to home care assignments may be required.

Qualifications

• Current, unrestricted RN license in state of practice.
• First Aid and BLS certifications required prior to hire.
• 1+ year experience working with frail or elderly populations.
• For assignments that include in-home care: personal transportation, valid state driver’s license, and auto insurance as required by law.

Responsibilities

• Perform comprehensive assessments, reassessments, and ongoing monitoring; update Plans of Care.
• Implement provider orders, including medications, treatments, and wound care.
• Triage calls to determine priority and route/escalate appropriately.
• Educate participants and caregivers on conditions, medications, equipment, safety, and disease management.
• Coordinate outside specialist visits, diagnostics, and hospital admissions as needed; arrange transportation when applicable.
• Complete timely documentation in the medical record; support compliance and quality initiatives.
• Participate in IDT meetings, family conferences, and care reviews.
