Job summary

InnovAge seeks a skilled Registered Nurse to provide both clinic- and home-based nursing care to frail and elderly participants. You will work with the interdisciplinary team to develop and implement individualized care plans and ensure safe, effective treatment delivery in both settings.

Responsibilities include comprehensive assessments, medication administration, care coordination, and thorough documentation. This role requires on-call coverage and travel to participant homes as assigned.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Current First Aid and BLS certifications (required prior to hire).
• 1+ year experience working with frail or elderly populations.
• For in-home assignments: valid driver’s license, auto insurance, and acceptable driving record.

Responsibilities

• Perform comprehensive assessments, reassessments, and ongoing monitoring; update Plans of Care.
• Administer medications, manage treatments, wound care, and other skilled interventions.
• Triage calls, prioritize, and coordinate care with providers and IDT.
• Educate participants and caregivers on conditions, meds, equipment, and safety.
• Coordinate outside visits, diagnostics, and hospital admissions; arrange transportation when needed.
• Document accurately in the medical record and ensure regulatory compliance.
• Support quality management, utilization review, and safety initiatives.
