Job summary

InnovAge in the United States offers in-home nursing services under the PACE program. The RN in this role provides skilled nursing care in participants’ homes or facilities, performs clinical and administrative tasks, and adheres to InnovAge policies and regulations.

Key duties include triaging calls, scheduling appointments, administering medications, documenting patient records, coordinating care with therapists, and educating participants and families.

Qualifications

• Associate’s degree in nursing required.
• Current State issued Registered Nurse license required.
• Current First Aid and BLS certifications required prior to hire.
• Personal transportation, valid driver's license, good driving record and auto insurance as required by law.
• 1+ year working with the frail or elderly.

Responsibilities

• Triage participant and staff calls and determine priority of care.
• Schedule outside appointments for participants as needed.
• Provide and assist in examination, testing and treatment of participants.
• Administer medications and treatments and document in medical records.
• Coordinate with home care team to ensure appropriate level of care.
• Educate participants and families on medications and treatment instructions.
• Supervise and mentor nursing staff as applicable.
• Refer to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker as needed.
