Job summary

InnovAge is seeking an In Home Services CNA to provide routine care and assist participants with activities of daily living in their homes or at InnovAge facilities. You will follow care plans, document care, and work with the nursing team to support independence and safety.

The role requires a current state CNA license, First Aid and BLS before hire, reliable transportation, a minimum of one year experience with the elderly, and a high school diploma or GED. Bi-lingual candidates are preferred.

Qualifications

• Current and valid state CNA license.
• Current First Aid and BLS certifications prior to hire.
• Require personal transportation, valid driver’s license, and adequate auto insurance.
• Minimum of one year experience working with the frail or elderly.
• High School Diploma / GED or relevant work experience in lieu of education.

Responsibilities

• Follow care plans and assist participants with activities of daily living (bathing, dressing, grooming, eating, toileting).
• Document and report care provided in accordance with policies and regulations.
• Monitor health status, vital signs, and basic needs during visits.
• Assist with meals, feeding, ambulation, positioning, transfers, and skin care.
• Provide in-home nursing assistant services and attend required trainings and QM activities.
