Job summary

InnovAge Holding Corp. seeks a Home Services CNA to provide routine care and assist participants with daily living activities in homes or InnovAge facilities. You will follow care plans, document care, and work with the RN and clinical team to support health goals.

Responsibilities include taking vital signs, assisting with feeding, transfers and skin care, and reporting changes promptly. Must have valid CNA license, First Aid, and BLS; a driver’s license and one year of elderly care experience

Qualifications

• Current and valid state CNA license is required.
• First Aid and BLS certifications required prior to hire.
• Personal transportation with valid driver’s license and auto insurance as required by law.
• Minimum of one year experience with frail or elderly populations.

Responsibilities

• Provide routine care and assist with activities of daily living in home or facility.
• Follow plan of care and document care provided according to policies and regulations.
• Take vital signs and monitor health status; report changes to RN.
• Assist with feeding, transfers, skin care, ambulation and other aides as assigned.
• Complete visit records timely and participate in care planning with multidisciplinary team.
