Job summary

InnovAge is seeking an In-Home Services RN to provide skilled nursing care in participants' homes or at InnovAge facilities. The role requires an Associate’s degree in nursing and an active RN license, with First Aid/BLS certifications before hire.

Expect travel to participant homes, driving, and coordination with the care team. The position emphasizes patient-centered care, documentation, and education for participants and families.

Qualifications

• Associate’s degree in nursing.
• Current RN license issued by the state.
• First Aid and BLS certifications required prior to hire.
• Must have personal transportation and valid driver’s license.
• At least 1 year of experience with frail or elderly.

Responsibilities

• Triages participant and staff calls to determine care priority.
• Schedules appointments and coordinates care with PCPs.
• Provides skilled nursing care including medications and treatments.
• Documents and maintains medical records and charting.
• Educates participants and families on medications and treatment.
• Coordinates care with home care resources and therapists.

Education

Associate’s degree in nursing

Current State issued Registered Nurse license

First Aid and BLS certifications

Personal transportation & valid driver’s license

1+ year experience with frail/elderly

Bachelor’s degree in nursing
