Job summary

Pinnacle Fertility is seeking a compassionate Patient Navigator to serve as the primary contact for patients throughout their fertility journey. This full-time role in Chandler or Glendale, AZ involves guiding patients, coordinating care, and collaborating with a multidisciplinary team to ensure exceptional support and service.

The position requires healthcare experience, strong communication, EMR proficiency, and empathy.

Qualifications

• Education and healthcare experience aligns with role.
• Minimum 2 years in customer service or patient navigation or related healthcare role.
• Bilingual skills are a plus.

Responsibilities

• Guide patients through treatment plans and educate on procedures.
• Coordinate appointments, tests, and referrals for seamless care.
• Communicate with providers, staff, and admin to coordinate care.
• Oversee treatment coordination between departments (financial, embryology, genetics).
• Maintain accurate EMR documentation and patient records.
• Provide emotional support and resources to patients and families.
• Participate in team meetings and quality improvement initiatives.
• Educate patients on treatment protocols, self-care strategies, and medications.
• Track progress, follow up on missed appointments, and ensure adherence to plans.
• Assist with after-hours call coverage on a rotating schedule.
