Pinnacle Fertility in Chandler, AZ is seeking a compassionate Patient Navigator to guide patients through their fertility journey. You will serve as the primary contact, coordinating treatment plans and educating on procedures, testing, and follow-up care, while working with a multidisciplinary team to ensure exceptional patient support.

This full-time role follows Monday–Friday hours, with training and travel required in Scottsdale during the first 12 weeks.

Qualifikationen

• High school diploma required; higher education preferred in healthcare.
• Bachelor’s degree in healthcare, social work, or related field preferred; CMA preferred.
• Experience with EMR systems and healthcare software beneficial.
• Strong knowledge of insurance processes and patient advocacy.
• Excellent communication, organizational and problem-solving skills.
• Bilingual skills are a plus; FDA 21 CFR Part 1271 experience preferred.

Aufgaben

• Serve as the primary point of contact for patients, guiding them through treatment plans and educating on procedures, testing, and follow-up.
• Coordinate and schedule patient appointments, tests, and referrals for seamless care.
• Communicate with healthcare providers and administrative teams to ensure coordination.
• Oversee treatment coordination between departments for each patient’s plan.
• Maintain up-to-date patient records in the EMR system.
• Provide emotional support and resources to patients and families.
• Participate in team meetings, case conferences, and quality improvement initiatives.
• Educate patients on treatment protocols, self-care, and medication usage.
• Track progress, follow up on missed appointments, and ensure adherence to plans.
• Advocate for patients’ needs and resolve barriers to care.
• Assist with after-hours call coverage on rotating schedule.
