Job summary

Pinnacle Fertility, a leader in fertility care, seeks a Patient Navigator to guide patients through their journey and coordinate with a multidisciplinary team. This on-site, full-time role in Grand Rapids, MI centers on compassionate patient support and education.

Ideal candidates have 2+ years in healthcare-related customer service or navigation, strong communication, and comfort with EMR systems. On-site responsibilities include after-hours call coverage and collaboration with clinical staff.

Qualifications

• High school diploma required; Bachelor's degree preferred.
• Minimum of 2 years in customer service, patient navigation, case management, or related healthcare role.
• Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
• Excellent communication, organizational, and problem-solving skills.
• Proficiency in using EMR and other healthcare software.
• Bilingual skills are a plus.

Responsibilities

• Guide patients through treatment plans and explain procedures, tests, and follow-up care.
• Schedule patient appointments, tests, and referrals to ensure continuity of care.
• Assist patients with understanding medical care, insurance coverage, and available financial resources.
• Coordinate with healthcare providers, clinical staff, and admin teams to ensure seamless care.
• Maintain accurate EMR documentation and track patient progress.
