Job summary

Citizens Options Unlimited in Syosset, NY is seeking a Direct Support Staff to provide ongoing guidance to individuals with intellectual and other disabilities, supporting self-care, health, safety, household chores and overall wellbeing.

You will help individuals become active in the community, assist with daily routines, transport to appointments, and participate in meetings while following treatment plans and agency policies; a valid NYS driver license is required and fingerprint/background

Qualifications

• Must have strong communication skills (written and verbal).
• Must submit a valid NYS driver’s license and maintain eligibility as a driver.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to promote independence and engagement in daily routines.
• Supports individuals in community participation and building natural supports.
• Assists with household chores and meal preparation; ensures health and safety.
• Follows treatment plans and uses adaptive equipment as needed to promote health and safety.
• Administers medications as directed per agency policy.
• Documents progress and participates in inter-disciplinary team discussions.
• Provides transportation to medical appointments and community activities.
• Mentors new staff and participates in staff trainings and meetings.
• Performs other duties as assigned by supervisor.
