Responsibilities include following treatment plans, supporting medication administration per AMAP, documenting progress, and collaborating with the interdisciplinary team. A NYS driver license and high school diploma are required.

• Provides ongoing guidance and support to individuals with disabilities to ensure active participation in daily life.
• Assists with self-care routines, health, safety, and household chores.
• Supports community participation and builds natural supports and relationships.
• Documents treatment plans and progress notes; follows safety equipment and AMAP medication procedures.
• Transports individuals to appointments and activities as needed.
• Mentors new staff and participates in house meetings and trainings.
