Job summary

Citizens Options Unlimited in Seaford, NY is seeking a Direct Support Professional to provide daily guidance and support to individuals in the Residential Services program, helping maintain a high quality of life through self-care, health, safety, and wellbeing.

Responsibilities include assisting with meals, transportation, medication administration per AMAP policy, and documenting plans and progress to promote independence within an inter-disciplinary team. A NYS driver's license is required.

Qualifications

• High school diploma or equivalent preferred.
• Must submit a valid NYS driver's license and maintain qualified driver status.
• Must be able to communicate and understand written and oral directives.

Responsibilities

• Provides ongoing guidance and support to individuals to ensure active participation in daily life.
• Supports daily self-care, health, safety, and household chores.
• Helps individuals become active community members and maintains natural supports.
• Follows treatment plans and assists with medications per AMAP policy.
• Documents care plans, medical notes, and progress to promote independence.
• Offers transportation for medical appointments, shopping, and outings.
• Mentors new Direct Support Staff and participates in House Meetings.
