Job summary

OneWell Health Care in Paxtonia, PA is seeking a dedicated Caregiver/Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. This role focuses on daily living activities, safety, and community participation while promoting dignity and independence.

The DSP will implement ISP/IPP plans, assist with transportation and personal care, and maintain accurate documentation.

Qualifications

• High school diploma or equivalent.
• Preferred: caregiving/home health/direct support for IDD.
• Ability to use EVV systems and maintain documentation according to guidelines.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health and safety.
