Job summary

OneWell Health Care is seeking a dedicated Caregiver / Direct Support Professional (DSP) in Burnham, PA. This field-based role provides hands-on, person-centered support to individuals with IDD in home and community settings, following ISP/IPP and behavioral plans.

The DSP will assist with daily living activities, health monitoring, documentation, and community participation while promoting independence and dignity. Strong communication and HIPAA compliance are essential.

Qualifications

• High school diploma or equivalent required.
• English proficiency in speech and writing required.
• Valid driver’s license may be required depending on assignment.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD.
• Assist with daily living activities and health monitoring per care plans.
• Maintain accurate documentation and HIPAA-compliant records.
• Support community integration and transportation as needed.
