Job summary

OneWell Health Care in Ambler, PA seeks a dedicated Caregiver / Direct Support Professional (DSP) to support individuals with IDD in home and community-based settings. You will provide hands-on care, help with daily living activities, and promote skills toward independence.

This field-based position requires reliability, strong communication, and ability to follow care plans, with documentation and team collaboration. A compassionate, patient approach is essential and training is provided.

Qualifications

• High school diploma or equivalent is required.
• English proficiency is required (spoken and written).
• Valid driver's license may be required depending on assignment.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community-based settings.
• Assist with personal care, meal preparation, mobility, and medication administration as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Promote independence and community participation by maintaining structured routines and supporting employment or vocational activities.
• Maintain accurate documentation, participate in trainings and team meetings, and adapt to staffing needs while complying with regulatory standards.
