Job summary

OneWell is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Franklin, PA. This field-based, non-exempt role supports individuals with IDD and Autism in home and community settings, promoting independence and dignity.

You will assist with daily living activities, mobility, meals, and medication under ISP/IPP guidance. Responsibilities include documenting care, participating in trainings, and coordinating transportation to appointments.

Qualifications

High school diploma or equivalent is required.
Experience in caregiving, home health, or direct support for individuals with IDD preferred.
Ability to follow written care plans and documentation guidelines.

Responsibilities

Provide hands-on, person-centered care in home and community settings.
Assist with daily living activities and personal care.
Support meal preparation, nutrition, mobility, and medication assistance as authorized.
Implement behavior support strategies and monitor health, safety, and emotional well-being.
Maintain structured routines and documentation, including service notes and shift verification.
