Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) in Pennsylvania to provide hands-on, person-centered support to individuals with IDD in home and community settings. The role emphasizes independence, dignity, and inclusion, with structured routines and opportunities for community participation.

The DSP will work under supervision, implementing service plans, documenting care, and coordinating transportation.

Qualifications

• High school diploma or equivalent.
• English proficiency (spoken and written) required.
• Driver's license may be required depending on assignment.
• Experience in caregiving, home health, or direct support preferred.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community-based settings.
• Assist with daily living activities, meal preparation, mobility, and medication as authorized.
• Support transportation to appointments and community outings.
• Maintain documentation including daily service notes and ISP/IPP compliance.
• Participate in trainings and team meetings and ensure HIPAA compliance.
