Qualifications

• CNA listed on Minnesota Registry.
• Strong communication skills with elders.
• Ability to work collaboratively in a team.
• Commitment to preventing abuse and protecting dignity.
• Ability to follow established care plans and safety procedures.
• Preferred: experience in long-term care, assisted living, or skilled nursing.
• Preferred: knowledge of EMR systems.
• Preferred: experience in household model of care or Green House homes.

Responsibilities

• Provide personal care and assistance with ADLs (bathing, dressing, grooming, toileting).
• Observe and respond to residents’ needs while promoting independence.
• Maintain confidentiality and privacy of resident information and documentation.
• Communicate effectively with coworkers and interdisciplinary teams.
• Participate in CST meetings to discuss resident progress and needs.
• Document and report changes in health, behavior, or nutrition.
• Follow infection control and safety protocols.
• Assist with safe transfers, positioning, and mobility support.
• Support restorative nursing and rehab programs as per plan.
• Measure and record vital signs, weight, and height.
• Assist with meals and fluids according to dietary plans.
