Qualifications

Responsibilities

• Provide ADL assistance (bathing, dressing, grooming, toileting).
• Observe and report changes in health or behavior.
• Maintain privacy and comply with documentation procedures.
• Assist with transfers and mobility using proper equipment.
• Support meals and hydration according to dietary plans.
• Follow infection control and safety protocols.
• participate in CST meetings and care plan updates.
