StoneBridge Senior Living is seeking a Certified Nursing Assistant to support nurses in providing compassionate care and daily living assistance to residents. The role emphasizes safety, documentation, and collaboration with a dedicated care team.

Ideal candidates have CNA certification in good standing, a state-approved training background, and strong communication skills. Priority is given to those with LTC experience and CPR/BLS credentials.

Qualifications

• Certified Nursing Aide in good standing with the certification board or graduate of a state-approved CNA training program and passed a state-approved competency examination within 120 days of employment
• High School Graduate/GED preferred
• Current CPR/BLS certification preferred
• At least one year of experience as a CNA in long-term care preferred
• Able to express self in written and oral communication in an interdisciplinary care setting

Responsabilités

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned
• Assists resident with Activities of Daily Living per care plans and policies
• Assists with lifting, turning, moving, positioning, and transporting residents
• Coordinates dining room services at mealtimes, including setup, delivery, and feeding assistance
• Delivers nutritional supplements and documents intake
• Assists with weighing residents and reports changes to nurse/supervisor
• Monitors skin condition and reports issues to nurse/supervisor
• Assists with toileting program activities
• Completes flow sheets daily and documents progress
• Keeps nurse and supervisor informed of factors affecting performance
• Reports changes in resident condition to the nurse/supervisor
