Responsibilities include assisting with daily living activities, documentation of meals and weights, and coordinating dining services while upholding policies. Strong communication with staff, residents, and families is essential.

Qualifications

• Certified Nursing Assistant certification or state-approved training program completion.
• CPR/BLS certification preferred.
• Minimum one year of experience in long-term care preferred.
• Ability to communicate effectively in written and oral form within an interdisciplinary care setting.

Responsibilities

• Provides supportive services to nursing staff and performs duties as assigned.
• Assists residents with activities of daily living per care plans and procedures.
• Assists with transferring, lifting, turning, and positioning residents.
• Coordinates dining room services at mealtimes and assists with feeding.
• Delivers nutritional supplements and documents intake.
• Records resident weights and reports changes to nurse/supervisor.
• Monitors skin condition and reports issues.
• Assists with toileting program activities and completes flow sheets daily.
• Keeps staff informed of factors affecting care delivery.
