Job summary

StoneBridge Senior Living is seeking a Certified Nursing Assistant to support residents and assist nursing staff in Adams St. The role requires patience, compassion, and a dedication to gentle elder care.

The CNA will help residents with daily living activities, follow care plans, document progress, and report changes to the nursing team. Relevant state-approved CNA program and CPR/BLS certification are required or preferred.

Qualifications

• Certified Nursing Assistant in good standing with state board or approved program.
• High School Diploma or GED preferred.
• CPR/BLS certification preferred.
• At least 1 year experience as CNA in long-term care preferred.
• Strong written and oral communication in interdisciplinary care setting.

Responsibilities

• Support nursing staff and perform duties as assigned.
• Assist residents with activities of daily living per care plans and procedures.
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Coordinate dining room services during meals; deliver trays and assist feeding.
• Deliver nutritional supplements and document intake; assist as needed.
• Record weights and report changes to nurse; document weight changes.
• Monitor skin condition and report issues to nurse and supervisor.
• Assist with toileting program activities.
• Complete flow sheets and document resident progress daily.
• Keep supervisor informed of factors affecting ability to perform tasks.
• Report changes in resident condition promptly to nurse and supervisor.
