Qualifications

• Certified Nursing Assistant in good standing or graduate of state-approved CNA program with competency exam completed within 120 days of employment.
• HS diploma or GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of long-term care experience preferred.
• Strong written and verbal communication in interdisciplinary care settings.

Responsibilities

• Provides supportive services to nurses and staff as needed.
• Assists residents with activities of daily living per care plans and policies.
• Assists with lifting, turning, moving, positioning, and transporting residents.
• Coordinates dining room services during meals and documents intake.
• Delivers nutritional supplements and monitors intake.
• Records resident weights and reports changes to nurse/supervisor.
• Monitors skin condition and reports concerns to staff.
• Assists with toileting and personal care activities.
• Completes daily flow sheets and documents progress as directed.
• Keeps supervisor informed of any factors affecting performance.
