Qualifications

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assists with lifting, turning, moving and positioning of residents.
• Coordinates dining room services at meal times and documents intake.
• Delivers nutritional supplements and records intake.
• Records resident weight and reports changes to nurse.
• Monitors skin condition and reports issues to nurse.
• Assists with toileting program activities.
• Completes flow sheets daily to show task progress.
• Keeps nurse informed of factors affecting work duties.
