Job summary

PACS Group is seeking a licensed Certified Nursing Assistant to provide routine daily nursing care to residents in alignment with individualized care plans. You will identify residents, assist with daily activities, monitor health, and document care while collaborating closely with nursing staff.

Responsibilities include accurate charting, reporting changes promptly, and maintaining a warm, safe environment that respects resident dignity and privacy.

Qualifications

• Must possess a valid Certified Nursing Assistant license in the state.
• Ability to read, write, and follow care plans and charts.
• CPR certification preferred and training provided as required.

Responsibilities

• Provide routine daily nursing care per resident care plans.
• Identify residents using wristband or photo ID before procedures.
• Record entries accurately on flow sheets, notes, and charts.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Assist with daily activities, meals, hygiene, and mobility.
• Maintain resident room readiness; greet and orient residents and roommates.
• Document food intake and monitor hydration; ensure water is available.
• Protect resident privacy and confidential information at all times.
• Participate in nursing shift reports and follow policies on exposure to blood and bodily fluids.
